Right-click on the link above, choose “Save Target As,” then choose a location on your computer to save it.

To use Rate Finder

1. Open the file from where you saved it locally on your computer.
2. When the macro alert pops up, click “Enable Macro.” The calculator will not work unless macros are enabled.
3. Choose a family structure to quote.
4. Choose the subscriber’s gender.
5. Choose the subscriber’s age.
6. Enter a Health Factor then press “Enter” or “Tab” to calculate.
7. To view, click the “Summary” tab at the bottom, or click the “Print Summary” button in the upper right corner.

The approximate monthly rates for all plans available (based on demographics entered) will appear on the summary page. If not, click the “Reset Form” button at the top of the form and try again.
