  • Discuss general provider issues
  • Receive information on how to change your office address or Tax ID
  • Receive information on how to add a practice associate or office location
  • Request a fee schedule

To update a provider's office address or Tax ID number, or add an additional practice address, please complete a Provider Information Update Form and submit it online, or you may mail or fax the completed form to Provider Services.

  • Request an agreement (contract) application to become a participating provider
  • Check on the status of your application
