Signature stamps no longer accepted
The Centers for Medicare & Medicaid Services (CMS) no longer accepts physicians' or other health care professionals' signature stamps in field 31 of the CMS-1500 claim form. CMS-1500 forms with the provider's name and credentials preprinted in field 31 are still acceptable. If the provider's name and credentials are not preprinted on the form, the provider must sign his or her name and add his or her credentials (e.g., MD, DO, PA, NP).
