Federal Employee Program (FEP) care coordination, support
when it's needed
The Federal Employee Health Benefit Plan is pleased to offer several
care management programs available to meet the health and wellness
needs of your Federal Employee Program (FEP) patients.
Our care managers are available to coordinate care for a variety of
health conditions, including:
- Acute care for cancer, stroke and surgery
- Chronic health conditions support for diabetes, asthma and heart
disease
- Co-care management for medical and mental health
- Complex outpatient coordination for multiple providers
- Drug and alcohol problems, including alcohol, drugs and prescription
medications
- End-of-life support with care planning and hospice
- High-risk maternity for multiple births, premature birth and pregnancy
after age 35
- Management of multiple medications for reducing unwanted side-effects
- Mental health treatment for depression and anxiety
- Pain management for chronic health and injury-related pain
- Assistance locating resources available within the community
