Innova®
and Engage®
Our Innova and Engage products are available to any employer groups. They feature flexible options; allowing employers to choose copayment, coinsurance and deductible amounts. Members can seek services from any provider.
Innova is ideal for individuals and families that may seek medical care several times a year. Innova includes:
- Differing benefit levels based on the member’s choice of provider
- Specific benefits are not subject to a deductible; as well as benefits that include copayments, deductibles and coinsurance
- Upfront benefits (not subject to deductible or coinsurance):
- Office visits: Office visits are subject to only a copayment (for Preferred and Participating providers only).
- Outpatient radiology and laboratory services: The first $400 of services from a professional, independent laboratory or facility (excluding inpatient services) per calendar year is covered at 100% of the allowed amount.
- Expanded office services optional benefit. When this optional benefit is purchased by the employer group, the following services (including related supplies) performed in the provider’s office are subject to coinsurance only. The deductible and the office visit copayment do not apply:
- Anesthesia
- Office surgeries
- Therapeutic injections
Engage offers simplicity to our members. Engage includes:
- Differing benefit levels based on the member's choice of provider
- Most benefits are subject to a deductible

Benefit structure and provider networks
These products use our established provider networks for the highest benefit levels. Innova and engage have categories of benefit levels.
Members can seek services from any provider. The benefits are designed to encourage members to seek services from Preferred or Participating providers. A member’s benefit level is determined by his or her choice of provider and the services received.

* Utah employers can select either the Preferred BlueOption Network or Preferred ValueCare Network as the Preferred network for their employees. When services are received from providers participating with the selected network, claims are processed at the Category 1 benefit level.
