option available to eligible beneficiaries in
Box Elder, Cache, Davis, Iron, Morgan, Rich,
Salt Lake, Summit, Tooele, Utah, Wasatch, Washington
and Weber counties in Utah.

- Regence MedAdvantage provides all
Part A and B benefits with additional
covered services in one plan. Most professional
services are covered at 100% after a copayment,
or paid in full for in-network providers.

- No Referral Requirements: Members
can seek services from any provider on
the Regence MedAdvantage network.

Identifying members
Regence MedAdvantage
member cards include:
- The Regence BlueCross BlueShield of Utah
(BCBSU) logo and the Medicare Advantage
PPO suitcase logo on the front
- The member number and a unique three
letter alphabetic prefix of ZVU. The full member
number, including the prefix must
be submitted on claims.

all services listed on the Medicare Products Pre-authorization
List.

If you are providing services for a patient
with a Medicare Advantage Private Fee-for-Service
(PFFS) plan, you can access the Medicare Advantage
PFFS terms and conditions
using the Web
Finder Tool. Although Regence BCBSU does
not offer a PFFS plan, other Blue Cross and/or
Blue Shield Plans (Blue Plans) do, and their
members may seek services in Utah. As with
other BlueCard members, PFFS claims should
be sent to Regence BCBSU.

It is important to use the correct Regence form based upon your geographic
location. Use of another health Plans notification form for Regence members
is not considered valid by CMS.
